We are your Home for Patient-Centered Care

A patient-centered medical home is a team approach to providing total health care using evidence based guidelines. Your medical home team will include your healthcare provider, others who support you and, most importantly, you.

Our Role as Your Provider

  • Provide you with personalized care because we know you and your health history.
  • Provide a safe environment to discuss confidential concerns.
  • Give a full explanation of any service or treatment provided.
  • Coordinate your care from our office, another physician’s office, hospital or other healthcare services.

Your Role as a Patient

  • Be an active participant with your health care team. Bring a list of concerns and questions to discuss with your provider.
  • Provide a list of all medications, including supplements, and over-the-counter medicine, so we can help prevent negative reactions.
  • Let us know as soon as possible if you had to go to the emergency room or urgent care center.
  • When you go for care elsewhere, tell them the name of your doctor and authorize to have your information sent to us, so we can keep it with your medical record.
  • Be informed about and involved in your healthcare decisions. Be sure to ask any questions related to your care, condition or treatment.
